This large observational study of the European Spondyloarthritis Research Collaboration Network (Eurospa) combining data from registries of 14 european countries, including SCQM, aimed at quantifying the impact of lifestyle factors on the response to a treatment in axSpA patients. A total of 16’696 axSpA patients that started their first TNF inhibitor treatment were included in the analysis. The association of lifestyle factors and treatment outcome was assessed by logistic regression. Smoking and being overweight or obese was associated with a decreased response to TNF inhibitor treatment. The effects were strong with an Odds ratio of 0.77 for smokers vs non-smokers and 0.55 for obese vs normal-weight patients.

Relevance:
The strong effects of smoking and being overweight on therapy responses to TNF inhibitor treatment in a very large collection of patients in this real-world study suggests, that rheumatologists should motivate patients to stop smoking and refer them to weight management interventions. Modifying these factors has a considerable potential to optimize treatment responses in axSpA.
